| Both students and managers are required to fill out an evaluation one month before the end of each work rotation. The Student Evaluation must be completed by the student before the manager is able to complete the Manager Evaluation. Evaluations serve to provide feedback on both ends about the program and student performance. | This is a form that is emailed to co-op students while they are at school and must be submitted one month before they may return to work each semester. |
